Non NHS Services

Private Work Fees

Some services provided are not covered under our contract with the NHS and therefore additional charges will be applied.

Examples include the following:

  • Medicals for pre-employment, sports and driving requirements (HGV, PSV etc.)
  • Insurance claim forms
  • Prescriptions for taking medication abroad
  • Private sick notes
  • Vaccination certificates

The fees charged are based on the British Medical Association (BMA) suggested scales and our Patient Services Team will be happy to advise you about them along with appointment availability.

You will be expected to settle any bills before any forms or papers will be released. Please note that we do not accept credit or debit cards, only cash, cheque or bank transfer.

Please note, non-NHS paperwork is secondary to the work the GP has to do on a daily basis and may be delayed due to GP workload.

If you have a form that needs completing, please bring it to the surgery or email us a copy. If you wish to request a letter, please use the form below.

Private Referrals

If you choose to be referred to a private provider for healthcare, there is no charge for the actual referral letter. However, you will have to pay for your healthcare at the private service, either through existing insurance, or from your own pocket. Please check you can pay for your consultations and treatment before requesting a private referral.

If your insurance company requests a form to be completed by the GP in addition to completing the referral, a charge will apply. Please email us a copy of the form and we will let you know the fee for completion.
